​Medication errors reduce the effectiveness and increase the potential harm of medications. Half of all medication errors occur at the interface between primary and secondary care. 
Murray is a medication expert. For people who require repeat prescription reviews he is the ideal clinician to do your review. Papamoa Pines is lucky to have Murray for these reviews not only because they optimise patient care but they reduce the appointment burden for the remaining GPs who then have more appointments available to see patients who have new symptoms or who are sick.
He originally trained as a pharmacist and has undergone additional post-graduate study to allow him to prescribe medicines in discussion with your GP. He has recently completed his Masters in Clinical Pharmacy and is involved in ongoing research and study.
During a consultation with Murray you may wish to ask any questions about your current medicines.
He can help to explain any changes to your medicines that may have happened from your GP or if you have recently been in hospital or seen a specialist.
He will work closely with your usual GP to ensure you are taking the correct doses of your medicines and the most appropriate medicines for you medical conditions.
He will review ALL of the medicines you may be taking and reconcile your medicines, which means resolving any discrepancies, discussing changes with patients to clarify any misunderstandings, updating records, provide medicines education and ensure an appropriate follow up plan is in place for monitoring including blood tests, adverse effects, clinical review and dose titration. This will mean the next time you see your GP your medicines list will be kept up to date. At Papamoa Pines we value teamwork.
