He will work closely with your usual GP to ensure you are taking the correct doses of your medicines and the most appropriate medicines for you medical conditions.
He will review ALL of the medicines you may be taking and reconcile your medicines, which means resolving any discrepancies, discussing changes with patients to clarify any misunderstandings, updating records, provide medicines education and ensure an appropriate follow up plan is in place for monitoring including blood tests, adverse effects, clinical review and dose titration.
