For the RAT Support Line call My Covid Record on 0800 222 478 and Press 3. This number is only for those requesting a RAT test or who want help to report their results on https://mycovidrecord.health.nz/

The Case Self Service Support Line is 0800 358 5453 and select option 2 
Also on option 2 is advice on using the online Self Registration Form.
